This clinic aims to improve the quality of life for seniors with complex healthcare needs. Seniors will have a two-hour comprehensive assessment with a physiotherapist, occupational therapist, nurse and social worker. We will assess balance, walking, thinking skills, level of function with self-care at home, social situation, mental health and physical health. The team will develop recommendations based on your goals/concerns and the results of the assessment, and share these with your doctor.

Anyone above 65 years of age who is:
